  SACRAMENT PREPARATION REQUIREMENTS
  
  By registering your child with Mary, Queen of Heaven, for Confirmation Preparation you affirm the following by checking the "I agree" box:
- I understand my PSR student(s) need to submit the Archdiocese of Cincinnati emergency medical, liability, and media release form prior to the first class.
  I understand that I am required to attend one of the parent information meetings.
  I am a parishioner registering my child, who is also a parishioner of a MQH parish, for the Confirmation preparation program. If you and/or your child are not a parishioner or any of any of the three churches, you have received proper permission from your pastor and Mrs. Barbara Yoder (Director of Evangelization) to be part of our sacrament.
  Your child has been baptized and has received First Communion in the Catholic Church, and you will be asked to provide a baptismal record at the parent orientation meeting or by November 23rd, if it has not been provided to their parish school.
  Your child practices their Catholic faith regularly and your family makes Sunday Mass attendance a priority.
  You are aware of the schedule of classes, retreat, and practice for Confirmation, and will be prioritize preparation for the period in which it takes place, understanding that repeated absences could result in the delay of the sacrament of Confirmation.

  SPONSOR REQUIREMENTS & EXPECTATIONS
  
  Requirements:
  Sponsor is NOT a parent, stepparent, or adoptive parent of the candidate
  Sponsor is at least 16 years old
  Sponsor is a registered and active member of a Catholic Parish
  Sponsor is not under any canonical impediments to being a sponsor (such as a currently in a marriage that is not recognized by the Catholic Church)
  Sponsor has received the sacraments of Baptism, Eucharist, and Confirmation
  
  Expectations:
  Attend Confirmation practice and the Confirmation Mass
  Fill out the Confirmation Sponsor Form, sign and return it.
  Make time for an interview by the candidate; they have prescribed questions.
  Spend some one-on-one time with the candidate.
  Get involved in a service activity together.
  Pray for the candidate regularly

- Confirmation Retreat
  
  The parish family has contracted with Net Ministries to provide an exciting and inspiring retreat for our youth who are preparing for Confirmation. So that we know how many students plan to attend, we need everyone to register for the retreat with their Confirmation registration. Space for the January retreat is limited to 90; if the retreat registrants hit the max number, another prayer and reflection gathering will be provided in lieu of the Net Ministries retreat.
